1. Technology Platform and Transportation Marketplace
SwiftMed is a technology platform and private-pay transportation marketplace designed to connect passengers, patients, families, caregivers, healthcare facilities, and other users with participatingtransportation providers.
SwiftMed facilitates the connection and coordination between passengers and transportation providers through its website and mobile applications.
SwiftMed does not itself provide, own, operate, or control transportation vehicles and does not independently provide transportation services.
Transportation services are provided by independent transportation providers participating on the SwiftMed platform.
2. Independent Transportation Providers
Participating transportation providers are independent businesses and are not employees, agents, joint venturers, or partners of SwiftMed unless expressly stated otherwise in a separate writtenagreement.
Each transportation provider is solely responsible for:
● Maintaining all licenses, permits, registrations, and operating authority required by applicable law
● Maintaining appropriate commercial automobile and other required insurance
● Maintaining and operating safe and properly equipped vehicles
● Hiring, supervising, training, and managing its drivers and personnel
● Ensuring that its drivers satisfy all applicable licensing, background-check, training, and qualification requirements
● Complying with applicable federal, state, and local laws and regulations
● Providing the actual transportation service to the passenger
● Determining whether it can safely accommodate a particular passenger or transportation request.
SwiftMed does not guarantee that any particular transportation provider, driver, vehicle, or trip will be available.
3. No Medical Services
SwiftMed is a transportation technology platform and is not a medical provider, healthcareprovider, ambulance service, or emergency medical service.
SwiftMed does not provide medical treatment, medical advice, diagnosis, nursing care, clinical monitoring, or emergency medical services.
Transportation arranged through SwiftMed is intended for non-emergency transportation only.
If you are experiencing a medical emergency, call 911 oryour local emergency medical services immediately.
4. Transportation Types
Depending on location and provider availability, the SwiftMed platform may facilitate connections for:
● Ambulatory assistance transportation
● Wheelchair transportation
● Non-emergency stretcher transportation.
Not every transportation provider offers every transportation type.
Passengers are responsible for accurately describing their transportation needs when submitting a request. SwiftMed and participating providers may decline or cancel a transportation requestif the requested service cannot be safely or appropriately provided.
5. Passenger Responsibility
Passengers and individuals arranging transportation on behalf of passengers are responsible for providing accurate and complete information regarding:
● Pickup and destination location
● Date and time of transportation
● Mobility requirements
● Wheelchair or stretcher requirements
● Required assistance
● Passenger condition and transportation needs
● Any other information reasonably necessary to arrange appropriate transportation.
Providing inaccurate or incomplete information may result in a transportation provider being unable to accommodate the passenger.
6. No Guarantee of Availability
Submitting a transportation request through SwiftMed does not guarantee that transportation will be available.
Availability depends on participating transportation providers, geographic coverage, vehicle availability, scheduling, transportation requirements, and other factors.
SwiftMed does not guarantee that a ride will be accepted, available at a particular time, or provided by a particular transportation provider or driver.
7. Pricing and Private-Pay Services
SwiftMed operates as a 100% private-pay transportation marketplace.
SwiftMed does not accept Medicare, Medicaid, or private health insurance as payment for transportation services unless expressly stated otherwise.
Passengers are responsible for transportation charges applicable to their trip. Pricing may vary based on transportation type, distance, location, time, provider, availability, and other applicablefactors.
8. Information on the Website and App
SwiftMed makes reasonable efforts to provide accurate and current information through its website and applications. However, SwiftMed does not guarantee that all information will always be complete,accurate, current, or error-free.
Information provided through the SwiftMed website or application should not be considered medical, legal, financial, or professional advice.
9. Third-Party Services and Providers
SwiftMed may provide access to or facilitate connections with third-party transportation providers and other third-party services.
SwiftMed does not control the independent operations of these third parties and is not responsible for their acts, omissions, representations, services, policies, or conduct except to the extentrequired by applicable law.
Any transportation service provided by a participating transportation provider is subject to the provider’s applicable terms, policies, and responsibilities.
10. Passenger and Driver Interactions
SwiftMed facilitates connections between passengers and participating transportation providers. SwiftMed is not responsible for the independent conduct of passengers, drivers, or transportationproviders before, during, or after a transportation service, except to the extent such responsibility cannot legally be disclaimed.
Passengers and transportation providers are expected to treat one another respectfully and comply with all applicable laws and SwiftMed policies.
11. Limitation of Responsibility
To the fullest extent permitted by applicable law, SwiftMed shall not be responsible for circumstances arising from the independent transportation services provided by participating transportationproviders, including delays, cancellations, route changes, vehicle issues, driver conduct, passenger conduct, or inability to complete a transportation request.
Nothing in this Disclaimer is intended to exclude or limit any responsibility or liability that cannot legally be excluded or limited under applicable law.
12. No Emergency Transportation
SwiftMed is not intended to replace emergency medical transportation.
DO NOT use SwiftMed for emergencies.
If a passenger requires immediate medical attention or emergency transportation, call 911.
13. Changes to This Disclaimer
SwiftMed reserves the right to modify or update this Disclaimer at any time. Changes will become effective when posted on the SwiftMed website unless otherwise stated.
Your continued use of the SwiftMed website or applications after changes are posted constitutes acceptance of the updated Disclaimer.
