TRANSPORTATION PROVIDER POLICY & AGREEMENT

This Transportation Provider Policy & Agreement (“Agreement”) is between SwiftMed LLC (“SwiftMed”) andthe transportation company (“Provider”) registering to use the SwiftMed platform.

By registering for or using the SwiftMed Provider App, the Provider agrees to the following terms.

1- SwiftMed Marketplace

SwiftMed is a private-pay technology platform and transportation marketplace thatconnects passengers with participating transportation providers.

SwiftMed does not itself provide, own, or operate transportation vehicles. The Provider is responsible for providing the actual transportation services to passengers.

2- Transportation Services

Providers may receive requests for:

● Ambulatory Assistance

● Wheelchair Transportation

● Non-Emergency Stretcher Transportation

The Provider may only accept trips that it is legally authorized, properly equipped, insured, and qualified to perform.

3- Licenses & Compliance

The Provider is solely responsible for maintaining all licenses, permits, registrations, operating authority, vehicle inspections, and other approvals required to provide transportation in eachjurisdiction where it operates.

The Provider must immediately notify SwiftMed of any suspension, expiration, revocation, or material change to its required credentials.

4- Drivers

The Provider is solely responsible for hiring, supervising, training, and managing its drivers.

The Provider must ensure that every driver assigned to a SwiftMed trip is properly licensed, qualified, trained, and authorized to provide the requested transportation service.

Drivers remain the responsibility of the Provider and are not employees of SwiftMed unless expressly agreed otherwise in writing.

5- Vehicles & Insurance

The Provider is responsible for maintaining safe, properly equipped, registered, inspected, and insured vehicles appropriate for the transportation services offered.

The Provider must maintain all insurance required by law and provide proof of insurance to SwiftMed upon request.

6- Passenger Safety

The Provider is responsible for the safe transportation of passengers and for complying with all applicable safety, accessibility, and transportation requirements.

The Provider must not accept a trip when it determines that the passenger’s needs exceed its vehicle, equipment, personnel, licensing, or operational capabilities.

7- Passenger Information

Passenger information received through SwiftMed may only be used for legitimate transportation-related purposes.

The Provider agrees to protect passenger information and comply with applicable privacy and confidentiality laws.

8- Trip Acceptance & Cancellations

The Provider is responsible for reviewing transportation requests before accepting them.

Once a trip is accepted, the Provider is expected to make reasonable efforts to complete the trip as scheduled.

Repeated cancellations, excessive delays, no-shows, or failure to properly perform accepted trips may result in suspension or termination from the SwiftMed platform.

9- Private-Pay Transportation

SwiftMed is a 100% private-pay transportation marketplace.

The Provider understands that transportation arranged through SwiftMed is not automatically reimbursed by Medicare, Medicaid, or private health insurance.

10- Fees & Payments

The Provider agrees to the applicable SwiftMed platform fees, commissions, or other charges disclosed during registration or otherwise agreed upon between the parties.

The Provider remains responsible for its own taxes, employee compensation, business expenses, and other financial obligations.

11- Independent Business

The Provider is an independent transportation business and is not an employee, agent, partner, or joint venture of SwiftMed.

The Provider maintains responsibility for its own business operations, personnel, vehicles, drivers, and transportation services.

12- Incidents & Complaints

The Provider must promptly notify SwiftMed of significant accidents, injuries, safety incidents, serious passenger complaints, or other events involving a SwiftMed passenger or trip.

The Provider agrees to cooperate reasonably with SwiftMed regarding safety and service investigations.

13- Account Security

The Provider is responsible for maintaining the security of its SwiftMed account and must not knowingly share account credentials with unauthorized individuals.

14- Suspension or Termination

SwiftMed may suspend or terminate a Provider’s access to the platform for violations of this Agreement, safety concerns, regulatory issues, expired credentials, inadequate insurance, fraudulentactivity, or other legitimate business or compliance concerns.

15- No Guaranteed Trips

SwiftMed does not guarantee any minimum number of transportation requests, trips, passengers, or revenue.

Provider participation is non-exclusive unless otherwise agreed in writing.

16- Provider Acknowledgment


By clicking “I Agree” on the app or otherwise registering for or using the SwiftMed Provider App, the Provider confirms that:

● The information provided to SwiftMed is accurate

● It maintains all required licenses, permits, insurance, vehicles, and qualified drivers

● It will comply with applicable laws and regulations

● It is responsible for the transportation services it provides

● It understands that SwiftMed is a technology platform and transportation marketplace

● It agrees to comply with this Transportation Provider Policy & Agreement.
